What is Resource Navigation?
Resource Navigation is a vital service designed to bridge the gap between individuals and essential community resources. Our trained Resource Navigators, who are also Community Health Workers (CHWs), provide personalized case management to help individuals and families access a wide range of services, including:
- Food Insecurity Solutions: Connecting clients to food banks, nutrition programs, and other assistance.
- Mental Health Support: Referring individuals to counseling, therapy, and support groups.
- Healthcare Referrals: Guiding patients to appropriate medical facilities and specialists.
- Substance Misuse Services: Offering access to treatment programs and support networks.
- Educational Support: Assisting with tutoring, scholarships, and educational resources.
- Financial Literacy and Budgeting: Providing workshops and tools for financial stability.
- COVID-19 Prevention and Mitigation: Offering information and resources on staying safe and healthy.
Our Resource Navigators are trusted members of the community, making them uniquely qualified to understand and address the specific needs of those they serve. This trust enables them to act as effective liaisons between health and social services and the community, facilitating access to essential services and improving the quality and cultural competence of service delivery.

Eligibility for Resource Navigation Services
Our Resource Navigation program is specifically designed to support families in the community. To be eligible for these services, applicants must:
- Be part of a family unit, including single parents or guardians with children.
- Reside within the designated service area of our community resource center which includes Coos County and Upper Grafton County.
